义工卡

Volunteer Tag


雪隆各调整站请注意：

制作义工卡必须：

1. 呈上正本的初、中级上课证/高级证。

 2. 写在初、中级上课证/高级证的姓名必须与居民证（My Card）相同。

3. 附上一张不超过一年的护照型近照。

 4. 请在照片的背面写上自己的姓名和常去做义工的调整站。

 5. 请呈交 2 零吉制作费。

 没呈上完整资料者，一概不获发出义工卡。

Selangor & KL Adjustment Centres please take note:

Compulsory requirements for Volunteer Tag making:

1. Produce Original Copy of the Yellow/Advance Card.

2. Make sure the name written on Yellow/Advance card is according to NIRC (My Card).

3. Attach one recent (Not more than one year old) passport size photo.

4. Please write your name and your regular Adjustment Centre at the back of your photo.

5. Please pay RM 2.00 for making the Tag.

Tag will not be done if any of the above mentioned requirements is not adhered to.
